Hospice Admissions RN position in the Kansas City Metro, MO area is a full-time role focused on coordinating new patient admissions, conducting comprehensive assessments, educating patients and families, and partnering with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required qualifications include MO RN license, current CPR, driver's license, two years RN experience, and one year hospice experience.

Formación

• Current Registered Nurse license valid for practice in Missouri (required).
• Current driver's license (required).
• Current CPR certification (required).
• Two years of RN experience in a clinical care setting (required).
• One year of full-time hospice experience (required).
• Reliable transportation and automobile liability insurance (required).
• Ability to work independently and make sound clinical decisions (required).
• Strong written, verbal, organizational, and interpersonal skills (required).

Responsabilidades

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
