Job summary

Horan Hiring Solutions, LLC seeks a Hospice Admissions RN for the Kansas City Metro, MO (east) area. This full-time role coordinates admissions, performs comprehensive head-to-toe assessments, and educates patients and families to ensure a smooth transition into hospice care.

The position covers a 50-mile radius from the office with a Tuesday–Saturday 10am–7pm schedule. Missouri RN license, CPR, driver's license, and hospice experience are required.

Qualifications

• Current Mo RN license valid for practice.
• Current driver's license.
• CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and auto liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams for seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
