Job summary

Connectly Recruiting is seeking a Hospice Admissions RN for the Kansas City Metro, MO area. This full-time role covers a 50-mile radius from the office with a Tue-Sat 10am-7pm schedule, coordinating patient admissions and educating families as part of the interdisciplinary team.

The ideal candidate holds an MO RN license, CPR certification, a valid drivers license, and has 2+ years of RN experience with at least 1 year in hospice.

Qualifications

• Missouri RN license in good standing.
• CPR certification required.
• Valid drivers license and reliable transportation.
• 2+ years RN experience in a clinical setting.
• 1+ year hospice experience preferred.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to ensure seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and plan of care.
• Perform head-to-toe physical assessments.
• Complete all documentation accurately and on time.
• Perform other duties as assigned.
