Job summary

ANX Home Healthcare & Hospice Care is seeking an Admissions Registered Nurse (RN) to join our per diem team in the San Mateo and San Francisco County area. The Admissions RN will present hospice services to patients and families and perform admission visits with contract documentation.

The role requires CA RN license, CPR certification, and at least 1 year of hospice RN experience. A valid driver’s license and reliable vehicle are also required.

Qualifications

• CA RN license is current and in good standing.
• Current CPR card for Healthcare Providers.
• 1 year RN hospice care experience preferred.
• Valid driver's license with insured vehicle.

Responsibilities

• Present ANX Hospice Care services to patients and families for admission.
• Perform admission visits and complete admission assessment and contracts.
• Coordinate medications and DME for same-day delivery.
• Educate patients and families about hospice care and services.
• Participate in weekend on-call rotations and after-hours as needed.
• Document nursing assessments and plan of care with family involvement.
