Job summary

Suncrest Hospice is seeking an RN Field Admissions Specialist to complete hospice admissions and patient visits, orient new patients to the program, assist with paperwork, and perform thorough nursing assessments. You will drive across the Bay Area to meet patients in private homes, assisted living facilities, or hospitals, and work 10-hour shifts four days a week.

We value hospice experience, California RN licensure, reliable transportation, and a willingness to travel long distances.

Qualifications

• Minimum 1 year of hospice, SNF, or hospital experience.
• Willingness to travel across the Bay Area.
• Reliable transportation and valid California driver's license.
• Registered Nursing Licensure in California.

Responsibilities

• Pay range $55.00-$65.00 per hour

The Registered Nurse (RN) Field Admissions Specialist will complete hospice admissions and patient visits as needed. Admission nurses are responsible for orienting new patients to the hospice program, assisting the patient and caregiver with all the legal paperwork, and completing a thorough nursing assessment.

• The candidate is required to work 10-hour shifts, 4 days a week (Wednesday to Saturday 12PM - 1030PM)
• Candidate must be willing to travel across the Bay Area (Up to 150 miles per shift)
• The candidate must be willing to travel throughout the Bay Area. This position involves regular driving over long distances, so comfort with travel is essential
• Minimum 1 year of hospice experience, SNF, or hospital experience
• Ability to establish both hospice eligibility and diagnosis when patient is appropriate
• Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.)
• Reliable transportation, valid driver's license, and auto insurance
• Registered Nursing Licensure in the state of California
