Job summary

Mission Hospice & Home Care is seeking a Registered Nurse for after-hours coverage with weekend shifts in San Mateo, CA. The role supports admissions and coordination with the Team Manager for eligible hospice patients across San Mateo County and nearby areas.

Ideal candidates hold an CA RN license and have experience in hospice or acute care settings. The position offers weekend scheduling, with pay range reflecting after-hours differential.

Qualifications

• Associate’s or Bachelor’s degree in nursing from an accredited School of Nursing.
• Current license as a registered nurse in the State of California.
• Experience in acute care/home health and/or hospice setting preferred.

Responsibilities

• Make in-person nursing visits to patients as assigned, or as indicated.
• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the transdisciplinary team or outside providers/services.
