Job summary

Suncrest Hospice is seeking a Registered Nurse Field Admissions Specialist to manage hospice admissions and patient visits across the Bay Area. This role requires orienting patients, assisting with paperwork, and performing comprehensive nursing assessments.

The position features 10-hour shifts, four days per week (Wed–Sat, 2:00 PM–12:30 AM) with travel up to 150 miles per shift, and a comprehensive benefits package including medical, dental, and vision insurance.

Qualifications

• 1 year hospice, SNF, or hospital experience preferred.
• Willingness to travel across the Bay Area (up to 150 miles per shift).
• Strong assessment and documentation skills.

Responsibilities

• Complete hospice admissions and patient visits as needed.
• Orient new patients to the hospice program and assist with paperwork.

Details

The Registered Nurse (RN) Field Admissions Specialist will complete hospice admissions and patient visits as needed. Admission nurses are responsible for orienting new patients to the hospice program, assisting the patient and caregiver with all the legal paperwork, and completing a thorough nursing assessment.

• The candidate is required to work 10-hour shifts, 4 days a week (Wednesday to Saturday 2PM - 12:30AM)
• Candidate must be willing to travel across the Bay Area (Up to 150 miles per shift)

Qualifications

• The candidate must be willing to travel throughout the Bay Area. This position involves regular driving over long distances, so comfort with travel is essential
• Minimum 1 year of hospice experience, SNF, or hospital experience
• Ability to establish both hospice eligibility and diagnosis when patient is appropriate
• Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.)
• Reliable transportation, valid driver's license, and auto insurance
• Registered Nursing Licensure in the state of California
