This full-time role covers a 50-mile radius from the office with a Tue-Sat 10am-7pm schedule; the nurse educates families and performs comprehensive assessments to ensure a smooth transition into hospice services.

Qualifikationen

• Missouri RN license required.
• CPR certification required.
• Two years RN experience in a clinical setting.
• One year of hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Aufgaben

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
