Job summary

LRSolutions, LLC in the Kansas City metro area (MO) is seeking a full-time Hospice Admissions RN to coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and collaborate with the interdisciplinary team for a smooth hospice transition.

The position covers a 50-mile radius from the office with a Tuesday-Saturday 10am-7pm schedule; ideal candidates have hospice and RN clinical experience and hold a valid Missouri license.

Qualifications

• Active Missouri RN license.
• Current CPR certification.
• Two years RN experience in clinical care settings.
• One year full-time hospice experience.
• Reliable transportation and auto insurance.
• Ability to work independently and make sound clinical decisions.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to ensure continuity of care.
• Educate patients and families about condition and needs.
• Perform head-to-toe physical assessments.
• Complete documentation accurately and on time.
• Perform other duties as assigned.
