Job summary

Anchor Health in Antioch, CA is seeking an Admissions Registered Nurse to join our hospice team. The role focuses on presenting Anchor Health services to patients and families to facilitate admission of appropriate cases, collaborating with the interdisciplinary team.

You'll evaluate patient eligibility, gather data, and develop admission orders while maintaining professional competence and strong relationships with referring sources. We offer competitive pay and full benefits.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current CA RN license with CPR certification.
• Minimum of one year nursing experience; home care or hospice preferred.
• Excellent communication and problem solving skills; able to educate patients/families.
• Able to lift and transfer patients and work with varied schedules.

Responsibilities

• Present Anchor Health services to patients and families to support admission.
• Evaluate patients for hospice appropriateness and complete admission data.
• Obtain agreement for admission from referred patients and families.
• Analyze collected data to facilitate admission orders with the team.
• Build relationships with referral sources to ensure responsiveness.
• Maintain clinical competence and collaborate with care team.
• Educate patients and families about hospice and palliative care.
• Other duties as assigned.
