Anchor Health in San Diego is seeking an Admissions Registered Nurse to join our hospice care team, presenting Anchor Health services to patients and families to ensure appropriate admissions.

You will participate in information gathering, assess admission suitability, obtain agreement, and collaborate with the interdisciplinary team to coordinate care. This role requires CA RN licensure and CPR, with prior hospice/home health experience preferred.

Competenze

• Graduate of NLN-accredited nursing school.
• Current CA RN license and CPR certification.
• One year of nursing experience in the last three years or RN with BSN and current license.
• Two years’ experience preferred in public health, home care, or hospice nursing.

Mansioni

• Participate in admissions information gathering and education.
• Evaluate patients for hospice appropriateness.
• Obtain agreement for admission for referred patients.
• Analyze data to facilitate admission orders.
• Build referral source relationships to support high-quality care.
• Maintain clinical and professional competence.
• Develop internal relationships with admissions and patient care staff.
• Address questions about hospice and palliative care with patients/families.
• Other duties as assigned.
