Job summary

Anchor Health in Hayward, CA is seeking an Admissions Registered Nurse to present our hospice services to patients and families and to guide appropriate admissions. You will evaluate referrals, coordinate with the interdisciplinary team, and educate families about hospice and palliative care while maintaining strong relationships with community physicians.

Candidates must be CA-licensed RNs with CPR, and have excellent communication, presentation, and assessment skills; a valid driver's

Qualifications

• Graduate of a NLN‑accredited school of nursing.
• Current CA RN licensure and CPR certification.
• 1 year of nursing experience within last 3 years OR BSN with active RN license.
• 2 years of experience preferred in public health, home care, or hospice.
• Excellent observation, verbal and written communication, problem solving and math skills; strong presentation skills.
• Licensed driver with insured vehicle.

Responsibilities

• Participates in information gathering, analysis, education and form completion for admissions.
• Evaluates patients for hospice appropriateness via records review.
• Obtains agreement for admission of referred patients meeting criteria.
• Analyzes data to facilitate admission orders.
• Builds referral source relationships to support service expectations.
• Maintains clinical and professional competence.
• Develops internal relationships with admissions and patient care staff.
• Educates patients/families about hospice and palliative care.
• Other duties as assigned.
