This full-time role coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and collaborates with the interdisciplinary care team to ensure a smooth transition into hospice services.

Qualifikationen

• Current Registered Nurse license valid for Missouri.
• Current driver's license.
• Current CPR certification.
• Two years RN experience in a clinical setting.
• One year of hospice experience.
• Reliable transportation and auto liability insurance.
• Ability to work independently and make clinical decisions.
• Strong written, verbal, organizational and interpersonal skills.

Aufgaben

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication.
• Educate patients and families regarding condition, needs and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
