Qualifications

• Current unrestricted RN license in the state of operation.
• 2+ years in hospice, home health, or similar healthcare setting.
• Valid driver’s license with vehicle insurance and access to reliable transportation.

Responsibilities

• Assess potential patients for hospice eligibility and meet families to determine needs.
• Maintain confidentiality and safety of patient records.
• Complete admissions and patient care orders timely and accurately.
• Communicate with physicians, referral sources, family, and hospice team about new admissions.
• Update leadership on status of pending referrals and admissions.
