Job summary

Chapters Health is seeking an Admission Registered Nurse to assess patients at the patient’s residence or hospital and initiate care within our system. The role ensures a smooth handoff to Care Navigation and represents the organization professionally with patients, families, and facilities.

Requirements include an active RN license in the working state, at least 1 year of nursing experience (hospice/hospital preferred), and experience with EMR/EHR systems.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience working with an EMR/EHR system.
• Strong clinical assessment skills with background in emergency department or ICU.
• Bi-lingual in Spanish preferred; English proficiency required.

Responsibilities

• Assess patients in residency or hospitals and initiate care into Chapters Health System services.
• Ensure seamless transition to Care Navigation and act as a liaison with care teams.
• Explain services to patients and families, addressing questions and concerns.
• Obtain physician orders and initiate Plan of Care for immediate needs.
• Document per policy and maintain professional standards in all facilities visited.
