Elara Caring in Oklahoma City is seeking a Hospice Admission Registered Nurse to assess patients for hospice eligibility, educate families, and coordinate admissions. The role requires strong clinical nursing skills and compassion to support patients and families.

You will collaborate with physicians, document assessments, and ensure admissions comply with agency policies. Travel up to 50% and a valid RN license are required; home health experience preferred.

Qualifications

• Current state RN license.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Bachelor's).
• 1 year of clinical care experience.

Responsibilities

• Complete initial patient assessments to determine hospice eligibility and care needs.
• Educate patients and families about hospice services, care options, and resources.
• Collaborate with physicians and care team to develop plans of care.
• Coordinate the admission process for a smooth transition into hospice services.
• Provide compassionate nursing care and emotional support to patients and families.
• Document patient assessments, admission information, and care plans accurately and on time.
• Communicate patient needs and changes in condition to the care team.
• Ensure admission activities follow agency policies and regulatory requirements.
