Job summary

Care Initiatives Hospice is seeking a compassionate Hospice Admissions Nurse in Iowa. You will meet patients and families where they are, coordinate admissions, and provide visits to new or ongoing patients under our hospice program. The role includes four 10-hour shifts, Tuesday through Friday, 8:00 a.m.

to 6:00 p.m., and working with the IDT to tailor care plans. As an RN with hospice experience and CPR certification, you’ll collaborate with Central Intake, Clinical Manager, and Community

Qualifications

• RN license required and CPR certification are mandatory.
• Two years hospice or home care experience preferred.
• CPR certification required.

Responsibilities

• Provide nursing services to hospice patients through visits and coordination with families and caregivers.
• Establish relationships with community sources to expedite admissions and coordinate care.
• Assess patient needs and share findings with the RN-Case Manager and IDT.
• Visit ongoing patients, revise care plans, and communicate assessments to the team.
• Deliver clinical services addressing goals of the care plan.
• Prepare notes and maintain clinical records; inform physicians of changes.
• Coordinate services and resources for patients and families.
