Job summary

VNS Health in New York seeks a Registered Nurse to provide high-quality hospice care to patients and families under the hospice philosophy. You will work under moderate supervision and collaborate with an interdisciplinary team.

Responsibilities include assessing needs, planning and evaluating care, visiting patients at home, coordinating with physicians, and supporting bereavement and education. NY state RN license required; associates degree minimum.

Qualifications

• License and current registration to practice as a Registered Professional Nurse in New York State required
• Demonstrated computer skills may be required based on operational needs
• Bilingual skills may be required, as determined by operational needs

Responsibilities

• Assesses the physical, social, psychological, living conditions and pastoral care needs of the patient/family through professional knowledge and skills of observation, interviewing and physical examination on an ongoing basis.
• Establishes and revises the nursing component of the patient/family plan of care based on goals mutually acceptable to the patient/family and significant others and makes referrals to other services, as necessary.
• Coordinates, implements and evaluates the effectiveness and outcome of the care plan by ongoing assessment, planning, coordination, counseling, teaching and supervision.
• Travels to patients' homes and/or other facilities with varying environments to deliver direct care to the patient.
• Transports and utilizes required medical equipment and supplies to and from patient homes/care facilities, VNS Health offices and other locations.
• Provides nursing care in accordance with Agency policies and Standards of Nursing Practice.
• Assists patients with activities of daily living, including positioning, moving, transferring and lifting as appropriate.
• Visits clients every two weeks to assess condition and update the plan of treatment.
• Monitors the effect of the plan of care and informs physician and staff of changes in patient needs.
• Manages a caseload of patients and participates in interdisciplinary team meetings.

Education

Associate's Degree in Nursing

Bachelor's Degree in a health or human services field or Nursing
