Gentiva Hospice is seeking a compassionate Hospice Admissions Nurse to join our team in Texas. The role involves visiting patients in home, hospital, or facility settings to assess eligibility, guiding families through admissions, and coordinating with physicians and care teams to ensure a smooth start to hospice services.

Applicants should have 1+ year of hospice nursing, an active RN license for the state, CPR certification, and be comfortable working Monday–Thursday 10am–9pm with the ability

Qualifications

• 1+ year of hospice nursing experience.
• Active RN license in the state of employment.
• Ability to work Monday through Thursday from 10am to 9pm.
• Hospice experience and CPR certification are required.

Responsabilités

• Visit patients and families in homes, hospitals, or facilities to assess eligibility for hospice care.
• Explain hospice philosophy and services clearly, compassionately, and with clinical confidence.
• Guide patients and families through the admissions process for a smooth start to care.
• Collaborate with referring physicians, case managers, and hospital staff to coordinate transitions of care.
• Document clinical findings and support care planning using the nursing process.
