Job summary

Gentiva is seeking a Registered Nurse Admissions (RN) for hospice services in Harlingen, TX. As a Hospice Admissions Nurse, you will be the face of care, guiding families through the admissions process with clinical confidence and empathy.

Prior hospice experience is required, along with at least one year of general nursing experience. You will visit patients in homes, hospitals, or facilities, assess eligibility, and collaborate with physicians and care teams to ensure a smooth transition into

• 1+ year of general nursing experience
• Hospice heart: compassionate, empathetic care
• Strong assessment and communication skills
• Comfortable visiting patients in home/hospital/facility settings

Responsibilities

• Visit patients and families to assess eligibility for hospice care
• Explain hospice philosophy and services with clarity and compassion
• Guide patients through the admissions process and ensure smooth start to care
• Coordinate with physicians, case managers, and hospital staff
• Document findings and ensure regulatory compliance
• Collaborate with interdisciplinary team for timely admissions
