Job summary

Iowa City Hospice is seeking a compassionate Hospice Admissions RN to be the first clinical contact for patients and families entering hospice services. You will guide admissions, assess needs, and ensure a smooth transition to care, while collaborating with physicians, social workers, and the interdisciplinary team.

This full-time role emphasizes clinical excellence, compassionate communication, and autonomy in a trusted nonprofit setting focused on dignity and end-of-life care.

Qualifications

• Current Iowa RN license in good standing.
• Graduate of an approved school of nursing with current Iowa nursing license.
• Two years of home health or hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Proficiency with electronic health records and standard office software.
• Advanced Hospice and Palliative Nursing Certification by year three.

Responsibilities

• Respond promptly to hospice referrals and inquiries.
• Conduct comprehensive admissions assessments in homes, hospitals, and facilities.
• Evaluate patients for hospice eligibility per guidelines.
• Explain hospice services, philosophy of care, and patient rights.
• Collaborate with physicians and care teams to coordinate admissions.
• Obtain and review medical records and orders.
• Complete admission paperwork accurately and compliantly.
• Coordinate initiation of services with the interdisciplinary team.
• Develop initial care plans addressing patient and family needs.
• Educate patients/families about disease progression and symptom management.
• Facilitate communication among all stakeholders.
• Maintain timely, accurate documentation in the EMR.
• Participate in on-call or weekend admission coverage.
• Represent Iowa City Hospice professionally in the community.
