Job summary

Heart to Heart Hospice seeks a compassionate Admissions RN to join our team in Kentucky. You will conduct initial assessments, review referrals, and collaborate with physicians and the care team to determine patient eligibility for hospice services.

This full-time role requires an RN with an accredited nursing degree, active state license, and 1–3 years of clinical or hospice experience. We offer competitive pay, benefits, a company car for qualifying individuals, and mileage reimbursement.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current State Registered Nurse license.
• Current driver’s license and reliable transportation; auto liability insurance.
• 1–3 years RN experience in clinical care, hospice or home health preferred.

Responsibilities

• Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Review clinical referral information and respond to requests and inquiries as appropriate.
• Collaborate with Patient Care Manager and hospice physicians to assess eligibility for hospice services.
• Conduct initial meeting with patient and family to discuss hospice services and obtain consent for care.
• Provide initial comprehensive assessment of physical, psychological, social and spiritual needs of patient and family; initiate interventions.
• Develop initial care plan with goals and interventions based on nursing assessment; coordinate with attending HMD and core team.
• Maintain regular communication with Intake Coordinator and care managers to review patients and needs; address issues.
• Ensure timely submission of all documentation and comply with laws and policies.
• Perform other duties as required and ensure compliance with regulations.
