Job summary

AccentCare, Inc. is hiring an Inpatient Hospice Admissions RN serving as the Clinical Liaison VIPC at Memorial Hermann Texas Medical Center.

You will identify patient needs and facilitate seamless transitions into AccentCare Hospice services, coordinating with hospital and care teams to ensure compassionate, timely care. This role requires an RN with strong communication, relationship-building, and independent work capabilities, plus a valid driver’s license and access to a vehicle.

Qualifications

• One year of RN experience.
• Strong communication and relationship-building skills.
• Ability to work independently in a hospital environment.
• Passion for serving with dignity and empathy.
• Valid driver’s license and insured vehicle.

Responsibilities

• Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in‑person collaboration and communication.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals‑of‑care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.
