Job summary

Capstone Hospice is seeking an Admissions Nurse to join our clinical admission team in Georgia. You will be the first clinical impression for patients facing terminal illness, conveying comfort and assurance while coordinating admissions in the service area.

The role requires an RN with a compassionate, patient-centered approach. You will complete admissions Monday–Friday, 10am–7pm, and collaborate with physicians to develop initial care plans and ensure timely documentation and handoffs.

Qualifications

  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse to practice in the state of Georgia.
  • Minimum of 2 years as a registered nurse.
  • Active patient contact within the past 3 years, to include hospice.
  • Active CPR certificate.
  • Must have current valid driver's license, auto liability insurance and your own reliable transportation.
  • Must be able to provide evidence of annual TB test and other state required tests or examinations.
  • Covid vaccinated.

Responsibilities

  • Complete at least 1-2 admissions per day when needed.
  • Collaborate with physicians and other professionals to form and initial plan of care.
  • Complete documentation within 12-24 hours of admission visit.
  • Provide support and comfort according to individual patient's needs.
  • Work with caregivers and patients to assess goals of care.
  • Maintain accurate records.

As the Admissions Nurse, you are a member of the dedicated clinical admission team that is responsible for the admission process of referred patients in the Capstone Hospice service area. You are the first clinical impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature, knowledgeable in the caring for the terminally ill and excellent interpersonal skills are required to be successful in this role. The goal is to help patients live their final days with dignity and comfort.

In this role you will have the expectations of being able to complete an admissions Monday - Friday between the hours of 10am-7pm.

Requirements

Candidates must meet the following qualifications and experience to be considered:

  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state of Georgia

Position Factors:
  • Requires physical effort most of the day including, walking, exposure to humidity changes. Exposure to patient homes, including their families and pets within the home.
  • Exposure to different social, racial, cultural and religious modes.
  • Must possess sight/hearing senses or use of appropriate adaptive devices that will enable senses to function at a level to meet the essential duties of the position.
  • Exposure to mental and emotional stress of the patients, caregivers and families.
