Job summary

Capstone Hospice, LLC in Georgia is seeking an Admissions Nurse to join the dedicated clinical admissions team. You will be the first clinical impression for patients and families, conveying comfort and assurance as you manage admissions and plan care for terminally ill patients.

You must be a registered nurse in Georgia with at least 2 years of experience, strong communication, and proficiency with EMR and mobile tablets.

Qualifications

  • Graduate from an accredited school of nursing.
  • Current Georgia RN license.
  • Active CPR certification.
  • Covid vaccination.
  • Minimum 2 years as a registered nurse.
  • Active patient contact within the past 3 years, including hospice.

Responsibilities

  • Complete at least 1–2 admissions per day when needed.
  • Coordinate with physicians and other professionals to develop and initiate a plan of care.
  • Document admissions within 12–24 hours of the visit.
  • Provide support and comfort according to patient needs.
  • Collaborate with caregivers and patients to assess goals of care.
  • Maintain accurate patient records.

As the Admissions Nurse, you are a member of the dedicated clinical admission team that is responsible for the admission process of referred patients in the Capstone Hospice service area. You are the first clinical impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature, knowledgeable in the caring for the terminally ill and excellent interpersonal skills are required to be successful in this role. The goal is to help patients live their final days with dignity and comfort.

In this role you will have the expectations of being able to complete an admissions Monday - Friday between the hours of 10am-7pm.

Requirements

Candidates must meet the following qualifications and experience to be considered:

  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state of Georgia
  • Minimum of 2 years as a registered nurse
  • Active patient contact within the past 3 years, to include hospice.
  • Active CPR certificate
  • Must have current valid driver's license, auto liability insurance and your own reliable transportation.
  • Must be able to provide evidence of annual TB test and other state required tests or examinations.
  • Covid vaccinated
To be successful in this role you should have the following knowledge & skills:

  • Strong nursing assessment skills
  • Proficient in the use of electronic medical record and hand-held tablet
  • Proficient in English (understanding and articulation)

Responsibilities:

Position Factors:
  • Requires physical effort most of the day including, walking, exposure to humidity changes. Exposure to patient homes, including their families and pets within the home.
  • Exposure to different social, racial, cultural and religious modes.
  • Must possess sight/hearing senses or use of appropriate adaptive devices that will enable senses to function at a level to meet the essential duties of the position.
  • Exposure to mental and emotional stress of the patients, caregivers and families.
