Hospice of the Valley is seeking an RN Admission Coordinator for weekdays and every other Sunday in Tucson. The role focuses on developing relationships with referral sources and conducting patient assessments to ensure smooth transitions into our hospice programs.

Qualifications

• Graduate of accredited school of nursing.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.

• Ability to manage time and follow-through on duties.

Responsibilities

• Facilitates smooth transition and admission of patients from health care settings into Hospice of the Valley programs.
• Promotes Hospice of the Valley services and programs.
• Develops and maintains therapeutic relationships with referral sources.
• Plans care and intervenes effectively for patient admissions.
• Provides patient/family teaching and supports end-of-life processes.
• Creates timely, accurate documentation and collaborates with teams.
