Job summary

Vogrinc Search Group is seeking a Hospice Admissions RN for Kansas City Metro, MO (east). This full-time role coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and collaborates with the interdisciplinary care team to ensure a smooth transition into hospice services.

The ideal candidate holds MO RN license, CPR, and 2 years RN experience with hospice preferred; valid driver's license and reliable transportation are required.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions
• Collaborate with patient care teams to support seamless communication and continuity of care
• Educate patients and families regarding condition, needs, and care
• Perform head-to-toe physical assessments
• Complete all required documentation accurately and on time
• Perform other duties as assigned
