Job summary

Mission Healthcare in San Diego County is seeking an experienced Registered Nurse for home health and hospice. The role involves initiating and revising plans of care, coordinating patient care as Case Manager, and collaborating with physicians to ensure high‑quality, compassionate care for patients and families.

Qualified candidates will hold an RN license, have 1+ year acute care experience, and 1+ year home care/hospice exposure.

Qualifications

• Graduate from an accredited nursing program.
• Current state license as a Registered Nurse.
• Minimum of 1 year of acute care experience in an institutional setting.
• Minimum of 1 year of home care, preferably hospice.
• Valid driver’s license and reliable transportation.

Responsibilities

• Initiates the plan of care and revises as patient needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with patient and family involvement.
• Provides health care instructions per assessment and plan.
• Assists activities of daily living and promotes self‑sufficiency and comfort care.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Communicates with physician and documents changes.
• Teaches self‑care and health counseling to patients and families.
