Job summary

Enhabit Home Health & Hospice is seeking a registered nurse to provide in-home care coordination and continuous assessment for patients and families in the San Antonio area. The role supports the interdisciplinary care plan and adheres to physician orders and regulatory standards.

The ideal candidate has at least one year of RN experience, holds an active nursing license in the state, and is willing to drive as part of home health services.

Qualifications

• Must have at least one year experience as a licensed registered nurse (RN).
• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must possess a valid state driver license.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
