• Willingness to travel to patient homes (approx. 20% daily)

Responsibilities

• Deliver compassionate, patient-centered hospice care with an interdisciplinary team
• Conduct comprehensive hospice admissions assessments and create care plans
• Educate patients and families on hospice process and expectations
• Ensure accurate EMR documentation and compliance
• Initiate evaluations to other disciplines when indicated
• Work independently, but collaborate with team
• Travel to patient homes within assigned territory
