Qualifications

• Active RN license in PA and in good standing.
• Two years of RN experience with hospice care preferred.
• Ability to drive to patient homes with valid license.

Responsibilities

• Deliver compassionate hospice care in patients' homes.
• Respond to after-hours needs with urgent assessments and symptom management.
• Support patients and families during evenings, weekends, and crises.
• Coordinate care with an interdisciplinary team and document in EMR.
