Job summary

St. Francis Hospice (SFH) is seeking an Admissions Registered Nurse to determine eligibility and prepare the initial plan of care for referred patients and families. The RN will collaborate with the Intake Coordinator, Clinical Service Department and COO to ensure timely processing of referrals.

Requires ADN and a current Florida RN license; BSN preferred. Hospice, home health, or palliative care experience is favored. Generous benefits package and a $6,000 sign-on bonus are offered.

Qualifications

• ADN required for licensure as RN in Florida.
• BSN degree preferred but not required.
• Active Florida RN license is required.
• Hospice, home health or palliative care experience preferred.

Responsibilities

• Determine eligibility and prepare initial plan of care for patients/families.
• Collaborate with Intake Coordinator, Clinical Service Department and COO to process referrals.
• Serve as a resource for questions from health care partners and staff.
