Job summary

Advocate Health is seeking a part-time RN to admit patients to hospice services and coordinate care with the interdisciplinary team. You will assess patients, obtain informed consent, and verify prognosis in collaboration with medical directors and the IDT.

You will review insurance eligibility, develop care plans, and ensure appropriate medications and equipment are delivered. Strong communication and organizational skills are essential.

Qualifications

• RN license in state where practicing; BLS certification eligible within 6 months.
• Associate's Degree in Nursing is required.
• Typically 2 years of direct patient care experience expected.
• Excellent communication, organization, and decision-making skills.

Responsibilities

• Lead admission to hospice services and coordinate with the interdisciplinary team.
• Obtain patient history, perform assessments, and determine appropriateness of hospice care.
• Explain hospice services and obtain informed consent; verify prognosis with IDT and medical director.
• Review insurance/Medicare eligibility and support billing.
• Coordinate care plans and arrange supplies, equipment, and services.
• Communicate effectively with IDT and coordinate handoffs and referrals.
