Job summary

Providence at Home with Compassus is seeking a Hospice Admission Registered Nurse (RN) to assess patients for hospice eligibility and coordinate care plans. You will work with an Inter-Disciplinary Team to address initial patient needs and enable families to spend meaningful time together with their loved ones.

The role requires an active RN license in the state, at least two years of nursing experience (hospice preferred), a valid driver’s license, and current CPR/BCLS certification.

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.

Compensation

$40.90-$63.49- The specific rate of pay offered to a selected candidate will be determined based on relevant education, training, experience, geographic location, and other position-related factors. Compassus does not inquire about, consider, or rely upon an applicant's prior salary history in determining compensation. In addition to base compensation for the position, an employee working in this position may be eligible for pay differentials based on hours, shifts worked, or other criteria. They may also qualify for bonuses under applicable bonus programs or at Compassus’s discretion, as well as other forms of variable pay under relevant company policies.A summary of applicable pay programs will be provided during the offer process.
