Qualifications

• Must have a current RN license in the state.
• Minimum one year of experience as a professional nurse or a relevant bachelor's degree.
• Ability to travel 20% of the time, often with little notice.
• Willingness to maintain knowledge of information systems and software.

Responsibilities

• Relate information to team members to promote hospice access.
• Assess and meet community educational needs related to hospice.
• Monitor community perceptions of Bristol Hospice as a high-quality provider.
• Facilitate communication with agencies and referral sources.
• Complete hospice consults, evaluations, and admissions.
• Coordinate with leadership for care continuity.
