Job summary

Good Shepherd Community Care is seeking an Admissions Nurse Liaison to join the expanding Intake and Admissions team. Travel is required and care is provided in patients’ homes and residential facilities throughout Greater Boston and parts of Metrowest.

The role requires MA-licensed RN with at least two years in community health, oncology, hospice, or palliative care, along with strong communication skills. Scheduling is Monday–Friday, 9am–5pm, for a 40-hour work week.

Qualifications

• Must be a MA-licensed RN.
• Two years of relevant experience preferred.
• Excellent communication and interpersonal skills.
• Ability to coordinate admissions and liaise with hospital staff.

Responsibilities

• Explain hospice services to patients, families and referral sources.

Travel is required for this position. Care is provided in patients’ private homes and residential facilities throughout Greater Boston and parts of Metrowest.

Job Responsibilities

The Admissions Nurse Liaison (ANL) is responsible for ensuring a comprehensive and smooth admission process for patients being admitted to Good Shepherd Community Hospice. The ANL provides explanation of hospice services to patients, families and referral sources, conducts clinical assessments, coordinates admissions, and maintains professional relationships with referral sources and Good Shepherd clinical staff.

Schedule And Weekly Hours

Monday through Friday from 9 am to 5 pm (40 hours)

Education, Skills And Experience

• Licensed by the Commonwealth of MA as a Registered Nurse.
• At least two years of experience preferred in community health, oncology, hospice, or palliative care.
• Highly skilled at providing patient assessments and evaluations.
• Excellent communication and interpersonal skills
• Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families and referral sources.
• Excellent problem-solving skills
• Competency with accepted standards of hospice nursing practice.

Pay Range

$89,000 - $106,000: This number reflects an average starting range for this position. Individual pay is determined through a variety of factors including experience, education, certifications, and skills and can exceed the range.
