Job summary

Good Shepherd Community Care is seeking a Per Diem Weekend Nurse Liaison to provide hospice information to patients and families, perform clinical assessments, and coordinate discharge planning with hospital staff. This role requires RN licensure in MA and strong collaboration with hospital teams.

The position offers Saturday and Sunday shifts with flexible hours; compensation is per visit as part of a per diem arrangement in the Greater Boston area.

Qualifications

Licensed by the Commonwealth of MA as a Registered Nurse.
Two years experience in community health, oncology, hospice, or palliative care preferred.
Excellent communication and interpersonal skills.
Ability to collaborate with hospital staff, Good Shepherd colleagues, patients and families.
Highly skilled at patient assessments and evaluations.
Excellent problem-solving skills.
Competency with hospice nursing standards.

Responsibilities

Provide information about hospice services to patients, families, and hospital staff.
Conduct clinical assessments.
Coordinate hospital discharge plans.
Maintain professional relationships within hospitals, focusing on assigned hospital(s).
