Visiting Nurse Association of Texas is seeking an RN Admissions to perform initial home assessments and coordinate care for hospice patients in the Tarrant County area. You will be the first clinical contact, ensuring a smooth transition into hospice services.

The role requires 2+ years of RN experience, a current Texas RN license, and a valid driver’s license. You must be able to lift up to 50 lbs, pass CPR/TB/Hepatitis screening, and be willing to participate in on-call rotations.

Competenze

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current Texas State license as Registered Nurse and a valid Texas Driver’s License.
• Minimum of two (2) years RN experience in a clinical care setting; one (1) year hospice experience preferred.
• Strong interpersonal skills and knowledge of hospice regulations.
• Ability to lift up to 50 lbs, pass CPR/TB/Hepatitis tests, and background check.

Mansioni

• Perform initial home assessments and administer nursing care.
• Coordinate care with Interdisciplinary Team to ensure seamless transition to hospice services.
• Develop and document plans of care in coordination with assessments and infection control measures.
• Maintain accurate clinical documentation in compliance with regulations.
• Meet productivity standards (2–3 admissions per day) and participate in on-call rotation.
