Job summary

VITAS Healthcare is seeking a healthcare sales professional in Bradenton, FL to support patient intake and admissions, and to develop consultative relationships with assigned accounts.

Responsibilities

• Assist in information gathering, education, and form completion for patient intake and admissions.
• Develop consultative relationships within assigned accounts through effective contacts and presentations.
• Evaluate patients for hospice appropriateness via medical records review as ordered by physician.
• Obtain agreement for admission of referred patients meeting criteria.
• Maintain competence in pain and symptom management guidelines and care coordination.
• Build relationships with senior management, admissions, and patient care teams.
• Support education activities with referral sources.
• Communicate effectively with customers individually and in groups.
