Job summary

AccentCare seeks a Director Admissions, Hospice in Clearwater, FL. This on-site leadership role oversees the admissions process from referral to admission, mentoring the Admissions Team and ensuring regulatory compliance.

Requires Florida RN licensure, a Bachelor’s degree, and 4+ years in hospice/home health or case management. Travel to all locations is expected; the position offers competitive benefits and 401(k) matching.

Qualifications

• Bachelor’s degree required.
• 4+ years hospice/home health or case management experience.
• Licensed RN in the state of operation (Florida).
• Must be a licensed driver who can travel to all locations.

Responsibilities

• Develops, coaches and serves as a resource and liaison to the Admissions Team from referral inquiry to admission.
• Ensure all patient and family services comply with federal, state and local regulatory requirements.
• Assume responsibility for a patient and family including assessing, planning, implementing and evaluating care.
• Coordinate with attending physicians, other hospice staff and agencies to optimize care and use of resources.
• Evaluate staff productivity and referral/admission data to ensure adequate staffing and budgetary requirements.
• Participate in site leadership meetings and DCO/TD meetings for operations feedback and ongoing education.
