Job summary

Hospice of Michigan in Kalamazoo, MI is seeking an Admissions Registered Nurse to coordinate and plan patient admissions and provide skilled nursing assessments as prescribed by the physician or designee.

The role requires MI RN licensure, a minimum of one year in general nursing (prefer hospice experience), and strong communication and organizational skills. You will work with families and care teams in hospital, home, and community settings.

Qualifications

• Graduate of an accredited school of professional (registered) nursing; BSN preferred.
• Current Michigan RN license.
• At least 1 year nursing experience with hospice; pain/symptom management and IV skills preferred.
• Maintains current BLS certification.
• Experience with terminally ill patients and pain management.
• Valid Michigan driver’s license and insured vehicle.
• Up-to-date immunizations per policy.

Responsibilities

• Coordinate and plan admissions and provide information to health care providers.
• Perform skilled nursing assessments as prescribed.
• Initiate plans of care and orders for medications/DMEs as prescribed.
• Document clinical notes and report to the Hospice Interdisciplinary Group after initial assessment.
• Provide initiation visits to patients and families in community/hospitals.
• Coordinate with insurance providers for benefits and payment options.
• Promote hospice referrals in the community and maintain community standing.
