Job summary
AccentCare in Michigan is seeking a Hospice Registered Nurse (RN) to join our interdisciplinary team. You will provide compassionate nursing care focused on comfort and dignity for hospice patients and their families, working Mon-Fri 8am-5pm with on-call rotation.
You will collaborate with physicians and care colleagues to create and carry out care plans, perform comprehensive assessments, educate patients and caregivers, and coordinate across disciplines to ensure continuity of care.
Qualifications
- RN license to practice in the state.
- Minimum of one year nursing experience.
- Reliable transportation and valid driver's license.
Responsibilities
- Provide individualized nursing care that maximizes comfort and quality of life.
- Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
- Perform comprehensive assessments to guide care decisions.
- Educate and support patients and caregivers, meeting them where they are.
- Coordinate care across disciplines to ensure continuity and dignity.
- Document care clearly and accurately reflecting patient needs and progress.
- Participate in on-call or admission rotations as needed.
- Engage in ongoing learning and professional development.
