Job summary

Desert Valley Hospice and Palliative Care is seeking an experienced Hospice Admission Nurse to evaluate and admit patients into hospice care. You will assess medical conditions, explain services to patients and families, and complete all required paperwork.

You will collaborate with physicians, social workers, and other care team members to create individualized care plans that prioritize comfort and emotional support during this transition.

Qualifications

• Active RN license in Arizona with fingerprint clearance.
• Current CPR certification.
• One year of hospice RN experience required.

Responsibilities

• Evaluate and admit patients into hospice care.
• Explain hospice services to patients and families.
• Complete all required paperwork for admission.
• Collaborate with providers, social workers and other healthcare professionals to create individualized care plans.
