Suncrest Hospice is seeking a Registered Nurse to provide compassionate in-home and facility-based care. You will work within an interdisciplinary team to assess, plan, and deliver patient-centered hospice services across homes, SNFs, and assisted living facilities.
The role may involve case management, admissions, or resource duties, with a focus on flexible collaboration, documentation, and ensuring high-quality care for patients and families in the service area.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
