Job summary

Suncrest Hospice is seeking a Registered Nurse Field Admissions Specialist to handle admissions and patient visits in the Bay Area. This role requires travel up to 150 miles per shift and 10-hour workdays from Wednesday to Saturday, 12 PM to 10:30 PM.

The position demands at least 1 year in hospice, SNF, or hospital settings, CA RN licensure, and reliable transportation. A strong orientation to patient and family education is essential for success.

Qualifications

• Minimum 1 year of hospice experience, SNF, or hospital experience.
• Ability to establish hospice eligibility and diagnosis when patient is appropriate.
• Willingness to travel to meet patients where they live (private homes, assisted living facilities, hospitals, etc.).

Responsibilities

• Complete hospice admissions and patient visits as needed.
• Orient new patients to the hospice program and assist with paperwork.
• Perform thorough nursing assessments and determine eligibility.
