Job summary

Jobtailor seeks a Patient Access Coordinator to manage referrals, admissions, and insurance verification across hospice and geriatric services. You will liaise with patients, families, and clinical teams to ensure smooth access and compliant data handling.

Role requires strong communication, DocuSign usage, and Epic proficiency, with on-site work and occasional emergency coverage. The candidate should have 3+ years in a health-care call center or similar setting.

Qualifications

• High School diploma or equivalent required.
• Associate’s or Bachelor’s degree preferred.

Responsibilities

• Serve as the primary contact for inbound calls, faxes, and emails from referral partners, community members, and Gilchrist staff.
• Coordinate referral development and patient access for hospice, palliative care, and geriatric medical services.
• Establish and maintain relationships with patients, families, and referral sources.
• Facilitate hospice admissions and appointment scheduling.
• Coordinate admissions across organizational disciplines.
• Verify Medicare/Medicaid and commercial insurance coverage.
• Coordinate concurrent care review with the clinical team as needed.
• Use DocuSign to obtain patient consent signatures and required documentation.
• Assist field staff with uploading documents to the EMR and obtaining consent signatures.
• Cross-train and fill in as Bed Coordinator.
• Monitor admission appointments for completeness, geographic efficiency, and nurse schedules.
• Assist with workflow revisions, Epic reporting, dashboard, and workqueue management.
• Manage data using Windows, Excel, Word, and Epic.
• Participate in mentoring and leadership development classes.
• Schedule visits, admission appointments, ambulances, and home medical equipment.
• Record referral information and maintain pending referral lists.
• Dispatch visits and coordinate schedules for providers, Access RNs, hospital liaisons, and pre-admission staff.
• Follow up with pending patients, non-admits, referral inquiries, and prospective patients.
• Process admission documents and enter patient demographics and status updates in Epic.
• Communicate updates with referral sources, internal stakeholders, DME providers, pharmacies, vendors, and admissions teams.
• Resolve referral and admissions issues and escalated unresolved concerns to the Access Center Manager.
• Mentor and train new team members and provide coaching to assigned mentees.
• Work Epic queues and run daily admission and referral reports.
• Ensure compliance with state, federal, and CHAP requirements.
• Generate and electronically file patient-related medical record information.
• Reverifying physician licensure and support special projects and process improvements
