Job summary

Low T Center is seeking Mid-Level Providers (NPs/PAs) to support expansion of their hormone replacement and targeted wellness programs across Illinois clinics. You will work with a physician director and center leadership to coordinate patient care, while maintaining high standards of service and compliance.

The role requires active Illinois NP/PA licensure, a current DEA license, and a CS license, with the ability to float among Hoffman Estates, Burr Ridge and Westmont locations.

Qualifications

• Active Illinois NP or PA license in good standing.
• Current DEA license.
• Current Controlled Substances license.
• Must float between Hoffman Estates, Burr Ridge and Westmont clinics.
• Must train Monday–Friday for 10 consecutive days.

Responsibilities

• Coordinate care with the physician medical director and Center Director.
• Manage patients receiving hormone replacement and targeted wellness.
• Operate in a fast-paced, customer-service focused environment.
• Maintain compliance with licenses and treatment protocols.
