Job summary

Low T Center is seeking a Mid-Level Provider to join our Illinois clinics and support hormone replacement and targeted wellness programs. You will work under a physician medical director and the Center Director to coordinate patient care and ensure high-quality outcomes.

Qualified candidates must hold an active Illinois NP or PA license, a current DEA license, and a current Controlled Substance License, with the ability to float between Hoffman Estates, Burr Ridge and Westmont.

Qualifications

• Must have active Illinois NP or PA license in good standing.
• Current DEA license.
• Current Control Substance License.
• Must be able to float between Hoffman Estates, Burr Ridge and Westmont Clinic.

Responsibilities

• Coordinate care for patients receiving hormone replacement and targeted wellness.
• Work with physician medical director and Center Director to oversee patient care.
• Support a fast-paced, patient-first environment with strong interpersonal skills.
• Provide two weeks of paid training and participate in clinic operations.
