SynergenX-Low-T in Indiana is seeking a Full-Time 4 day Float Mid-Level Provider (NP or PA) to deliver hormone replacement and targeted wellness care in a patient-centered, collaborative clinic. The role emphasizes patient assessments, individualized treatment plans, and education on wellness protocols, with a supportive team including the Center Director and Medical Director.

Shifts include Tue, Thu, Fri, Sat with clinic hours Mon 8–5, Tue 8–7, Wed 8–1, Thu 7–5, Fri 8–5, Sat 8–12; 32 hours per

Qualifications

• Active Indiana NP or PA license in good standing.
• Current DEA license or ability to obtain.
• Strong communication and clinical decision-making skills.

Responsibilities

• Provide hormone replacement, weight management, and targeted wellness care.
• Conduct patient assessments, create individualized treatment plans, and follow-up care.
• Educate patients on wellness, treatment protocols, and lifestyle optimization.
• Collaborate with Center Director, Medical Director, and clinic staff.
• Maintain accurate documentation and compliance with clinic protocols.
