Job summary

Parents and Children Together in Honolulu, HI is seeking a Parent Educator to support prenatal mothers and young families. The role focuses on evidence-based home visiting to improve maternal and child health, safety, and development outcomes; leadership within the service team; and collaboration with families, providers, and community partners.

Responsibilities include developing Family Goal Plans, administering developmental screens, documenting interactions, and linking families to resources,

Qualifications

• High School Diploma or equivalent required.
• Experience working with children and families required.

Responsibilities

• Conduct evidenced-based home visiting services to families with young children, ages 0-5 in a manner that aligns with funder/state guidelines and agency policies.
• Develop a meaningful Family Goal Plan (FGP) with families addressing risk factors and maternal-child health.
• Administer developmental screens (ASQ/ASQ-SE), hearing/vision screens, and parent assessments like AAPI/HOME.
• Enter and maintain contact documentation in program databases and funder systems accurately and timely.
• Provide linkage to community resources and follow-up with support services as needed.
